Read moreUSA Volleyball no longer requires the Medical Release form at its national events. For that reason, the Carolina Region does not supply the form to clubs any longer. Some clubs may choose to still collect a Medical Release form from their participants.
If a place for a notarized signature is part of that form, it would only be required if attending a state that does require signatures to be notarized.
